Lifeline

  • Low-income customers may apply for reduced cost wireline phone service through designated telephone companies
  • Enhanced Lifeline is available only on Tribal Lands
  • Basic Lifeline is available only on non-Tribal Lands
  • These programs combine with Link Up to provide low-income households with discounts for the installation of phone service (known as Link Up) and monthly phone bills (known as Lifeline) at their principal place of residence
  • May only be used as the primary telephone in a household. The name on the bill must match the member participating on the eligible program with the exception of the National School Lunch Free Program.
  • These programs are provided by a participating carrier (eligible telecommunications carrier or ETC)

Eligibility

  • Must be a receiving member of the following programs to receive Lifeline
    • Food Stamps
    • Medicaid (SoonerCare)
    • Supplemental Security Income (SSI)
    • Federal Public Housing Assistance/ Section 8
    • National School Lunch (free program only)
    • Temporary Assistance for Needy Families (TANF)
    • Low Income Home Energy Assistance Program (LIHEAP)
    • Vocational Rehabilitation (including Aid to the Hearing Impaired)
    • Oklahoma Sales Tax Relief
    • Bureau of Indian Affairs General Assistance
    • Tribally Administered TANF
  • Proof of eligibility is given by submitting a form signed under penalty of perjury that you receive benefits from or a copy of any dated document which verifies your participation in one of the qualifying programs.
  • Service may be terminated or moved to non-Lifeline status is the self-certification document is not received.
  • Eligibility is reviewed periodically. Benefits will be discontinued when household no longer meets requirements or when proof of eligibility is not received.
  • Customers who are no longer eligible for Lifeline benefits must notify their service provider.
  • Customers cannot be a dependent for federal income tax purposes, unless the applicant is more than 60 years of age.

Link Up

  • Link Up program reduces eligible low-income consumers’ charges for initiating telephone service by one-half of the telephone company’s charge, or $30, whichever is less.
  • For subscribers residing on tribal lands, the discount is up to $70 or 100% of the charges between $60 and $130, in addition to the $30 available to non-tribal subscribers.
  • A consumer may not receive more than one Link Up discount at the same address.
  • The Link Up discount can only be used for the charges for activating a primary telephone line or moving existing service to a new address.
  • The Link Up discount cannot be applied to charges for facilities or equipment that fall on the customer side of demarcation point (e.g. telephone, wiring inside the home, etc.)
    • Demarcation Point (D-Marc): the grey box, usually on the side of your home or apartment building. It is the place where your Telephone Service connects form the Phone Company to your residence. Anything before the D-Marc is considered outside the home and the responsibility of the Phone Company; anything after the D-Marc is considered inside the home and is the responsibility of the customer.
  • The Link Up discount cannot be used to offset the cost of a wireless handset.

The following services are also provided:

  • Voice Grade Access to the Public Switched Network: defined as a functionality that enables a user of telecommunications services to transmit voice communications, including signaling the network that the caller wishes to place a call, and to receive voice communications, including receiving a signal indicating there is an incoming call. Bandwith for voice grade access should be, at a minimum, 300-3,000 Hertz.
  • Local Usage: means an amount of minutes of use of exchange service, prescribed by the Commission, provided free of charge to end users.
  • Dual Tone Multi-Frequency signaling or its functional equivalent: is a method of signaling that facilitates the transport of signaling through the network, shortening call set-up time.
  • Single-party Service or its Functional Equivalent: is a telecommunications service that permits users to have exclusive use of a wireline subscriber loop or access line for each call placed, or, in the case of wireless telecommunications carriers, which use spectrum among users to provide service, a dedicated message path for the length of a user’ particular transmission.
  • Access to Emergency Services: includes access to services, such as 911 and enhanced 911, provided by local governments or other public safety organizations. 911 is defined as a service that permits a telecommunications user, by dialing the three-digit code "911," to call emergency services through a Public Service Access Point (PSAP) operated by the local government. "Enhanced 911" is defined as 911 service that includes the ability to provide automatic numbering information (ANI), which enables the PSAP to call back if the call is disconnected, and automatic location information (ALI), which permits emergency service providers to identify the geographic location of the calling party. "Access to emergency services" includes access to 911 and enhanced 911 services to the extent the local government in an eligible carrier's service area has implemented 911 or enhanced 911 systems
  • Access to Operator Services: defined as access to any automatic or live assistance to a consumer to arrange for billing or completion, or both, of a telephone call
  • Access to Interexchange Service: defined as the use of the loop, as well as that portion of the switch that is paid for by the end user, or the functional equivalent of these network elements in the case of a wireless carrier, necessary to access an interexchange carrier's network
  • Access to Directory Assistance: defined as access to a service that includes, but is not limited to, making available to customers, upon request, information contained in directory listings
  • Toll Limitation for Qualifying Low-Income Consumers: denotes either toll blocking or toll control for eligible telecommunications carriers that are incapable of providing both services. For ETCs that are capable of providing both services, "toll limitation" denotes both toll blocking and toll control.

Toll Limitation Services (TLS)

  • Toll Limitation Service (TLS) support allows eligible consumers to choose toll block or toll control at no cost.
  • TLS is a service that eligible telecommunications carriers must offer to eligible low-income subscribers at no charge. Qualifying low-income consumers choose whether or not they want TLS. This service includes toll blocking, which allows subscribers to block outgoing toll calls, and toll control, which allow subscribers, in advance, to limit their toll usage per month or billing cycle.

Tribal Lands Defined

  • Tribal Lands are defined as any federally recognized Indian Tribe’s Reservation (The Osage Nation) or former reservation in Oklahoma or an Indian Allotment.
  • Customer must certify sign under penalty of perjury on application that you are a resident of Tribal Lands to quality for Enhanced Lifeline
  • Areas of Oklahoma not eligible are the following counties:
    • Cimarron
    • Texas
    • Beaver
    • Greer
    • Harmon
    • Jackson
    • Area of Beckham County south of the North Fork of the Red River

Late Payments:
All late payments are subject to a late fee of five percent (5%) of the balance if not paid by the due date. If you do not pay my monthly bill on the due date, and do not give notice of cancellation to Dart Phone, a $10 Service Continuation Fee may be applied to your account to allow additional time to pay your monthly charges with Dart. Once your account reaches $75 or is 60 days past due, your service will be terminated and a $25 Service Termination Fee will be applied to your account as well as the late charge.

Wiring Responsibilities:
Dart’s automatic maintenance ends at the Network Interface Box. From the Interface Box to the inside of the customers premise is the responsibility of the end user or property owner. A $1.93 monthly line fee may be assessed to all accounts to cover existing equipment and wiring.

Former Cox customers:
Your home wiring may not be connected to the Network Interface Box. You, the customer, will be responsible for moving the wiring from the Cox Telephone Box back to the nearest Network Interface Box.

Annual Service Renewals:
Each year, consumers must renew their lifeline connection. Dart mails out renewal forms to consumers, and if the consumer fails to mail back the form, the lifeline benefit may be terminated.

Returned Check Policy
Dart reserves the right to charge up to $30.00 for each returned check. Dart Phone may, at its own discretion, re-deposit the check. Consumer understands that Dart Phone may not re-deposit the check automatically.
